We are professionals with decades of experience working in a variety of healthcare settings. Throughout those years we recognized an obvious disconnect between the services we know are available, and the patient's ability to access those services with ease and efficiency.

Too often we've seen overwhelmed and exhausted families trying to make the best decisions for people they love, without the necessary information or support. We have the knowledge and experience to make a difference in the lives of families, and from there, the Care Navigation program was created.
