Our Solution Based S.O.A.R. Plan is the foundation of every Nest In Place Program, providing clients and their families with a comprehensive solution for all their care needs including:
S: Support with access to professionals who will listen to your concerns, answer your questions, and problem solve as challenges arise. We carefully design and manage your in-home program, offering you flexible scheduling and NO HOURLY MINIMUMS.
O:
Organization for your appointments, home, meals, medications, coordination of care between medical providers.
A:
Advocacy talking to doctors and other healthcare providers, insurance, pharmacy, hospital case managers, even the cable or electric company.
R:
Resources including access to our list of trusted partners, our research and recommendations for equipment and services, education and guidance from experienced professionals.
Our Bird’s Eye View program is designed for those who need companionship, or help with household tasks such as shopping, laundry, activities, exercise, meal planning and prep, assistance with technology and more.
Bird’s Eye View is a great option if you just need help with certain activities so you can continue to thrive at home. There is NO HOURLY MINIMUM. This program is also a great starting point for anyone who is unsure if in-home care is the right fit. There is no contract.
Under Our Wing provides a deeper level of support. This program focuses on all your personal care needs, including bathing, dressing, walking, transfers, feeding, or medication reminders.
Our Post-Hospital program works closely with you and your medical team to facilitate a successful transition home after a hospital stay or surgery. This usually includes a schedule of hours that taper you recover and regain you independence.
Our Respite program is for family caregivers. Caring for a loved one can be stressful and overwhelming. With respite care, you can rest, attend social events, run errands and go to appointments while knowing your loved one in trusted hands.
In flight provides and enhanced level of care in situations where care solutions are provided by other professionals in a formalized setting such as personal care homes, nursing homes, or hospice.
In senior communities, nursing homes and during hospice admissions, there are times when clients require one-on-one individualized care. We work with the professionals overseeing your care to create a customized schedule that meets your care needs, whether it's a one hour check-in, or support for longer hours, up to 24 hours a day.
